The Community Health Assist Scheme (CHAS) enables all Singapore Citizens, including Pioneer Generation (PG) and Merdeka Generation (MG) cardholders, to receive subsidies for medical and/or dental care at participating General Practitioner (GP) and dental clinics.

  • All Singapore Citizen are eligible for CHAS.


Households can use this application to apply for the Community Health Assist Scheme (CHAS), which provides subsidies at participating CHAS GP and dental clinics.

Documents required

  • Valid Singpass to login to the e-Service.
  • NRIC 
  • Birth Certificate
  • FIN numbers, and contact numbers of all household members.
How to Apply CHAS?

To apply for CHAS. With this change, one household member may apply for CHAS on behalf of his/her household members.

  • Singapore Citizens who are 21 years old and above are able to apply for CHAS online here.
  • Download and complete the following hardcopy form: CHAS Application Form

What is the procedure to renewal CHAS Card?

  • Existing CHAS cardholders may have their CHAS cards automatically processed for renewal three months before expiry.
  • To reassess your household’s CHAS eligibility, will send you a renewal invitation package about three months before your card expires.
  • To submit your renewal for CHAS online by logging in with your Singpass account

How to report lost/damage replacement of card?

  • To replace your lost/damaged CHAS card, you may submit your replacement request here.
  • CHAS hotline at 1800-275-2427 to submit a request for a replacement.

  • Existing CHAS cardholders may have their CHAS cards automatically processed for renewal three months before expiry.
